Replacement is four jobs sold as one: demolition, haul-off, subgrade correction, and the repour. That is why replacement bids vary more than new-pour bids, and why an unitemized lump sum is impossible to evaluate.
The conditions that broke the original are usually still there after it is broken out. If the base was poorly compacted, or water was routed against the slab, or it was poured too thin for what parks on it, then pouring new concrete onto the same situation buys the same failure on a slower clock.
A contractor who can tell you what went wrong the first time is worth more than one who simply quotes the replacement. Ask directly.
Where the concrete is structurally sound but has settled — a sunken section, a lifted trip edge, a slab that has dropped at one corner — polyurethane foam lifting restores level for a fraction of replacement cost, in a day, with dime-sized holes.
Replacement is the honest answer where the concrete itself has deteriorated: surface spalling, widespread map cracking, crumbling edges, or exposed and rusting reinforcement. If a contractor quotes full replacement on a settled but intact slab without mentioning lifting, get a second opinion.
Demolition and haul-off as a separate line. Subgrade work as a separate line, with what is actually being done to it. The new slab with thickness, reinforcement and joint layout stated. Permits, if required, and who is pulling them. Four numbers you can compare, instead of one you cannot.

Technically yes, and it is occasionally appropriate — but as a general practice it is a poor idea. An overlay adds height (often creating problems at the garage door and at transitions), it bonds unreliably to a deteriorated surface, and it inherits every movement the old slab is still undergoing. Where the old concrete is sound and the height works, a bonded overlay can be reasonable. Where the old slab is cracking, it is a delay dressed up as a repair.
Demolition and haul-off on a typical two-car driveway is meaningful labor plus disposal fees, and it varies with thickness, whether the slab is reinforced (mesh and rebar slow demolition considerably) and site access. Insist on it as a separate line item — it is one of the largest sources of variation between replacement bids.
